It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please ask us about financing options, credit card, and debit card payments.

 

Please note, most insurance companies do not cover the refraction fee. Your estimated patient portion, including refraction fee, must be paid at the time of exam. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid in full. If you have any questions, our courteous staff is always available to answer them.

If special arrangements are needed, please talk to our financial counselor prior to receiving service. We accept most major insurances and, as a courtesy to you, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.

 

We ask that all co-pays and co-insurances be paid at the time services are rendered. HMO patients must be referred by their primary care physician and are responsible for obtaining a referral prior to the examination.
